Cancer Support Community Arizona (CSCAZ) is a nonprofit organization providing free psychosocial support to people diagnosed with cancer and their loved ones, family, and friends. We provide services based upon our Five Pillars: support groups, educational presentations, healthy lifestyles (e.g. yoga, mindfulness, expressive arts, nutrition, etc.),social connections, and resource and referrals (community supports to mitigate barriers to treatment).
If you wish to refer a patient or caregiver to our services, please first obtain their consent for us to outreach them. Then, submit this form and a representative from CSCAZ will contact them to initiate services.

Sunday: Closed (Except last Saturday of the Month for Rise & Recharge)
In June and July, our office will be closed on Fridays, but we will be available by phone.
